1. The Mandatory Certificate Dossier
During the physical reporting window (August 20–25 for MCC Round 1), candidates must carry two sets of self-attested photocopies along with the original documents arranged in the standard sequential order specified by medical admission scrutiny committees.
2. Master Document Organization List
- Provisional Allotment Letter: Downloaded from candidate portal after round result declaration.
- NEET UG 2026 Admit Card: With passport photo pasted and left-hand thumb impression.
- NEET UG 2026 Scorecard / Rank Letter: Showing candidate's All India Rank.
- Class 10 Certificate & Mark Sheet: Verifying Date of Birth and parentage spelling.
- Class 12 Mark Sheet & Passing Certificate: Showing individual and aggregate marks in PCB & English.
- Proof of Identity: Aadhaar Card, Passport, PAN Card, Voter ID, or Driving License (Must match application data).
- Passport Size Photographs: 8 to 12 identical copies matching the NEET application form.
- Category Certificate (SC/ST/OBC-NCL/EWS): Issued by designated revenue authority in prescribed Central/State format. (OBC-NCL & EWS must be issued on or after April 1, 2026).
- PwD Disability Certificate: Issued strictly by one of the 16 NMC-Designated Disability Assessment Boards.
- Domicile / Residence Certificate: Mandatory for all State Quota seats.
- Transfer Certificate (TC) & School Leaving Certificate: From last attended school/college.
- Migration Certificate: Required if shifting between university boards.
- Character / Conduct Certificate: Issued by the Head of the last attended institution.
- Gap Certificate / Affidavit: Stamp-paper affidavit if candidate dropped one or more academic years.
- Medical Fitness Certificate: Issued by a Registered Medical Practitioner (MBBS/MD) confirming clinical fitness.
